I hereby apply to receive water connection and/or garbage collection services from the City of South Jordan Utilities Department.

1. I, the customer, agree to permit South Jordan City, its agents or employees to enter the described premises at all reasonable times for the purpose necessary to render such service.
2. Customer agrees not to tamper with the meter or other meter equipment on their property. It is understood that any such actions will result in tamper fees and other equipment costs and/or charges to fix or replace equipment damaged by the customer’s actions.
3. Customer agrees to pay all utility bills on or before the due date of each month. It is understood that if the utility bill becomes 60 days delinquent, the service will be discontinued. In order for service to be restored, a reconnection fee will be charged and all delinquent amounts paid in full.
4. Customer agrees to notify South Jordan City of termination of service at least 48 hours prior to moving.
5. Customer agrees that they will make certain that the meters and equipment are readily accessible to the City and that there are no barriers or animals which would prevent reasonable access.
6. Customer agrees that they are the owner of the above listed property.
7. Any notices which are to be given to the customer shall be mailed, emailed, or both to the customer at the address listed on this agreement.
8. It is understood that all fees, interest, water rates or other financial charges can change at any time by action of the South Jordan City Council and will affect the customer as of the date directed in the actions of the City Council. All such financial charges are a part of the City’s annual budget process and can be located on the South Jordan website.
9. I hereby agree and understand I am financially responsible to pay for all charges rendered by South Jordan City. I understand that a service charge of 5% of the remaining balance shall be assessed and charged on all accounts delinquent over 30 days, with a minimum charge of $.50. I further understand that in the event a payment is not made at the time and in the manner required by the City, that I will be responsible for all costs of collection, including but not limited to attorney fees, court costs and collection charges up to 40% along with any commission that may be assessed by a collection agency and incurred by the City in collecting delinquent amounts. I agree, in order for the City or its collection agency to service my account or to collect any amounts due, that I may be contacted by telephone at any telephone associated with my account, including wireless telephone numbers, which could result in charges. I may also be contacted by sending text messages or e-mails, using any email address associated with my account. Methods of contact may include pre-recorded/artificial voice messages and/or use of an automatic dialing device, as applicable.
10. I understand that signing this form subscribes me to the email list for informational City emails (newsletter, safety information, public works information etc.) and will have the option to unsubscribe once the first email is sent to me.

This can be signed only by the property owner or his/her legal representative.

• New License Fee: $10 per dog.
• Renewals will be mailed yearly. Renewal Pricing:
  • Spayed/Neutered $5
  • Non-spayed/Unneutered $20

Per the City of South Jordan's Municipal Code:

1. Any dog required to be licensed shall be covered by homeowners, renters, or other insurance.
2. No person shall own or harbor more than two household pets per species over the age of four months per dwelling.
3. The City shall issue a license for a dog located in the city upon satisfaction of the following conditions:
   • Owner of the dog completes an approved application.
   • Owner of the dog pays the required fee (fee set by resolution of the City Council)
   • Proof of updated rabies vaccination or a notarized letter from a veterinarian is provided stating a dog cannot be vaccinated due to health reasons.
4. The owner of a dog shall at all times cause the dog to wear the license tag.
5. Please refer to City Code Chapter 6 for all ordinances and requirements for licensing/owning a dog in the City of South Jordan.
