I hereby authorize The City of South Jordan hereafter referred to as “City,” to take and publish photographs and/or videos taken on the date of event selected below of myself and/or the minor child or children listed below, and our likenesses, for use by the City in print, digital, or other mediums, as well as possibly in City publications.
I understand that I will not be paid, compensated or reimbursed in any way for the City’s current or future use of the photographs or likenesses. I further acknowledge and agree that I have no rights of ownership to the photos or videos and/or likenesses.
I understand that the City intends to place the photographs and/or videos on its website and social media, and make copies of the photographs available for the personal use of those in the photo.
I hereby release and hold harmless the City, its employees, and any third parties involved in the creation or publication of photographs and/or videos or likenesses from liability for any and all claims by me or any third party in connection with the photographs/videos taken on the date specified above. I knowingly give up any reasonable expectation of privacy or confidentiality for myself and for the minor child or children listed below (if any) associated with those images and/or videos.
Further, I attest that I am the parent or legal guardian of any child or children listed below (if applicable) and that I have full authority to consent and authorize the City to use their photos/videos and/or likenesses.
I am over the age of 18.
