**South Jordan will not process passport renewals as the U.S. Department of State does those conveniently online. Please schedule an appointment with us if you need a NEW passport or a passport photo.
ATTENTION: Please DO NOT use third-party companies or websites that charge additional fees to “help” complete your application. All applicants can fill out their forms directly at travel.state.gov at no cost. Passport fees are not paid until you are present at an official Acceptance Facility, and only the standard U.S. Department of State and execution fees apply.

Passport Application Requirements
- A Passport Application is required for each person applying for a passport. Applications must be filled out prior to arriving at your appointment.
- Each person applying for a passport, including minors, must be present.
- A Passport photo is required. South Jordan can take your photo for $15.00 per photo. Photo Requirements.
- A Valid Photo ID is required with your application, provide at least one of the following:
- Current driver's license (out-of-state license requires secondary ID)
- Aircraft pilot's license
- Official military ID card
- Passport
- Both Parent's identification for minor applicants (under age 16), unless Form DS-3053 (Statement of Consent: U.S. Passport Issuance to a Child) is provided
- Proof of Citizenship is required with your application, provide at least one of the following:
- Original birth certificate or certified copy issued by the county health department (hospital-issued birth certificates are not accepted)
- Original Certificate of Naturalization
- Original Certificate of Citizenship
- Previous U.S. passport book or card
- Checks are required for payment per applicant for the Department of State fees. Money orders are available for purchase ($3) at your appointment.
Additional Information
- Minors need to come in with their parent(s) or legal guardian(s) to apply for a passport.
- Both parents must be present unless one parent has filled out a State of Consent (DS-3053), or a guardian must provide proof of custody (sole legal and physical), death certificate or legal guardianship paperwork.
- If a parent has a different last name from the minor applicant (excepting maiden name) please provide a marriage license or court order showing the name change.
- Birth certificate for each minor.
- Provide each minor's valid passport if previously issued.
- For children under age 16, minor passport application fees apply.
- Minor Passports are valid for five years.
- Minors are able to apply independently of parents if they are able to submit sufficient photo ID and evidence of parental awareness (check from a parent or parent listed as emergency contact). One parent's attendance is required if either of these requirements are not met.
- Provide valid passport if previously issued.
- Original birth certificate or certified copy issued by the county health department (hospital-issued birth certificates are not accepted).
- For minors age 16 or 17, adult passport application fees apply.
- Adult Passports are valid for ten years.
Eligible U.S. citizens who are applying for routine service can now renew their passports online.
Visit the U.S. Department of State Site for up-to-date Adult Passport Renewal Requirements (i.e. was issued within the last 15 years).
If you are renewing online, South Jordan can still help you with updated passport photos. Money orders are not currently available, but will be at a future date.
